3. Photo consent form (I would like to save some paper and the process time this year so our consent form is changed.) Every year Union Chinese School take photos in varies of events. If you DO NOT wish UCS to use you and your children’s photo in any publications, website, facebook, and other media, please fill out the attached form. By not fill out the form, you automatically agree to allow UCS to use photo/video. any question, please feel free to contact Ichi Chen Hsu.
